The heart of Carrum Health is a seamless and highly personalized patient experience, enabled by expert care coordination and navigation. The Clinical Care Navigator III is a senior individual contributor who independently manages complex, clinically nuanced patient journeys. This role serves as a trusted operational and clinical partner, contributes to workflow refinement, and models best practices for clinical navigation and patient advocacy.

  • Independently manage high-complexity, clinically nuanced patient scenarios with minimal direction.

  • Apply advanced clinical judgment to coordinate care across multiple teams and touchpoints.

  • Ensure patient journeys remain aligned with clinical intent, operational execution, and patient needs.

  • Identify gaps in workflows and contribute to improving care continuity and navigation quality.

  • Support the development of CCN I and CCN II team members through mentorship and example.

  • Act as a role model for Carrum’s patient-first, clinically grounded approach to navigation.

Key Responsibilities:

  • Manage a moderate caseload (~100-125 complex cases) of patients requiring coordinated referrals, scheduling, and follow-up.

  • Independently manage a moderate-to-high complexity clinical caseload.

  • Coordinate care across clinical experts, operational teams, and external partners.

  • Ensure accurate, timely documentation and proactive follow-up.

  • Surface insights that improve clinical navigation workflows and patient outcomes.

  • Serve as a go-to resource for peers on clinical workflows and best practices.

Qualifications and Requirements: 

  • 4+ years of clinical experience in Substance Use Disorder and Behavioral Health patient care with an LCSW, LCPC, LSW, or LPC License with at least 3+ years of direct patient care or coordination experience 

  • Substance Use Disorder certification is strongly encouraged

  • Nice to have: Spanish Speaking preferred

  • Nice to have: Direct patient experience in broader behavioral health including eating disorders, other addictions (gambling, sex, digital), and acute mental health needs
