This ecosystem includes specialized tools for our providers and a fully accessible Member Portal (web and mobile) for our Members.
The Role:
Ilant Health is looking for a Peer Navigator to guide prospective bariatric surgery and obesity medicine patients through the process, answer questions, educate them with facts, and share your own experience navigating the process. This role will provide significant support to individuals who are pursuing ways to improve their health using clinically-supported strategies.
Key Responsibilities:
Conduct outreach to individuals who can benefit from the program to proactively enroll them
Conduct the initial virtual meeting for individuals exploring weight loss treatments, such as bariatric surgery and obesity medicine
Use Ilant tools and standard operating procedures (SOPs), based on clinical evidence and best practices, to support patients in learning more about the bariatric surgery journey
Provides ongoing support and education for patients via video calls, phone, email and text throughout their journey
Provides specialized documentation required to support insurance approval for bariatric surgery
Facilitate support groups
Qualifications:
You have lived obesity and treatment experience: you’ve undergone bariatric surgery or taken anti-obesity medication
Share personal experience while balancing it with factual information and the greater trends associated with bariatric procedures
You are comfortable conducting video calls and communicating via phone, email, text
Great communication skills
Supportive, empathetic and non-judgmental disposition
Experience with computers and software such as Google Workspace, Slack, etc.
Nice to haves:
Experience with Athena EHR, Talkdesk, and Zendesk are a plus!
Experience peer counseling or coaching previously
Licenses: LSW, Nutritionists, RN, Certified Coach
